Quarantine System for Entry (For Crew)
  1. 請於航班抵臺後，再進行線上健康申報
  2. 填寫系統前，請先確認防疫旅宿之訂房代碼、公司宿舍或符合「1人1室/1人1戶」檢疫規定(詳細內容，請按我)
  3. 抵臺後全程佩戴口罩，儘速前往檢疫處所且不得搭乘大眾運輸。搭乘防疫車隊、入住防疫旅宿時，主動出示本通知書收執聯。
  4. 除執行飛航任務(限派飛長程航班)外，應留在檢疫處所，不得外出。前往啟程地機場應搭乘公司專車或防疫車隊。
  5. 自主詳實記錄體溫及健康狀況及配合必要之關懷追蹤機制(包含持臺灣手機門號進行個人活動範圍之電子監督)。
  6. 如有發燒、咳嗽、腹瀉、嗅味覺異常或其他任何身體不適，請佩戴醫用口罩，立即通報所屬航空公司，並主動與當地衛生局聯繫(或撥1922)，依指示儘速就醫，且禁止搭乘大眾運輸工具就醫。
  7. 本系統適用於機組員防疫作業辦法經民航局備查之國籍航空公司。
  8. 本人同意檢疫通知書及提審權利告知，以電子表單方式收執。
※ 國籍航空公司機組員，自「重點高風險國家」航線航班返臺後，應居家檢疫及檢疫期滿進行PCR檢測，且應入住防疫旅宿或符合規定之公司宿舍。
  1. Please make an online health declaration after your flight arrives in Taiwan.
  2. Before filling up the system, please confirm the the quarantine hotel’s reservation code 、company accommodation or comply with the quarantine requirement of “One person per residence”.
  3. After arriving in Taiwan, you must wear a face mask all the time and go to the quarantine location as soon as possible. Do not take public transportation. Please present this notice voluntarily upon getting in a designated transport vehicle and checking in at the quarantine hotel.
  4. Except for performing flight duties (only duties for long-haul operations can be permitted), you must stay at your quarantine location; do not go outside. You must ride in a car arranged by your company or the designated transport vehicle to the airport of departure.
  5. Please record your body temperature and health status, and cooperate with caring and tracking measures (including using Taiwan’s cell phone signals to implement electronic monitoring of your location).
  6. If you have symptoms such as fever, cough or other discomfort, please put on a medical mask, report to your airline company immediately and contact with the local health authorities (or call the toll-free hotline 1922) to obtain instructions on seeking medical attention. Do not take public transportation when you go to the hospital.
  7. This system is intended to be used by Taiwanese Airlines implementing crew pandemic prevention practices accepted by the CAA.
  8. I agree to receive and keep a record of the COVID-19 Health Declaration and Home Quarantine Notice and Notice of Quarantine and Right to Petition for Habeas Corpus Relief in electronic form .
※ Air crewmembers of Taiwanese airlines, after returning to Taiwan from "key high-risk countries", shall undergo home quarantine and take a PCR test at the end of their quarantine periods; they shall also stay in a quarantine hotel or a company dormitory that meets related regulations.
